Your Questions Answered

Discharge planning should begin as early as possible – ideally from the moment you are admitted to hospital. Hospital staff will work with you, your family, and your support coordinator to prepare a safe and structured plan for your return home or into the community.

A discharge summary is a document prepared by hospital staff that outlines the treatment you received and the care required after leaving hospital. For NDIS purposes, this information must be translated into functional language that clearly explains what you can and cannot do in daily life due to your condition.

Yes. If your current home is not suitable or your support needs have changed, the NDIS may fund Medium Term Accommodation (MTA) while you wait for a long-term housing solution. In some cases, you may also be eligible for Specialist Disability Accommodation (SDA).

Your NDIS plan may include a range of in-home supports such as personal care, assistance with daily activities, support workers, home modifications (e.g. ramps or rails), and assistive technology to support your recovery and independence.

If your needs increase or decrease, you can request a review or change to your NDIS plan. This is done by submitting updated evidence from health professionals explaining why your supports need to be adjusted.

You can refer to the official NDIS guidelines on hospital stays and transitions, which explain how the health system and NDIS work together during discharge planning and support transitions.
